PASRR II Evaluations

​​If your facility is contracted with Rogue Psych Consultants:
Request that the current RPC provider assigned to the building complete the PASRR II evaluation. Please give the provider the PASRR I Form or completed Resident Review Form so they can proceed with the evaluation. Or fill out our request form and we will reach out to schedule an appointment.

If your facility is not contracted with Rogue Psych Consultants:
Please fill out our request form and we will reach out to schedule an appointment. Send patient records and PASRR I and/or Resident Review Forms to the fax or secure email below.
