PASRR II Evaluations for SMI

Rogue Psych Consultants conducts PASRR II evaluations for individuals with Serious Mental Illness (SMI).

A PASRR II evaluation may be requested following a PASRR I indicating potential SMI or a Resident Review referral based on a significant change in status related to serious mental illness.

PASRR I & Resident Review Guidance

PASRR I

The PASRR I is the initial screening used to identify individuals who may have a serious mental illness.

• PASRR I score of 3 or 4: Request a PASRR II evaluation from Rogue Psych Consultants.

• PASRR I score of 1: If the resident has no current symptoms or behaviors, no PASRR II is needed at this time.

• PASRR I score of 1, however the PASRR I is incorrect or the resident develops new or worsening psychiatric symptoms, behaviors, or other changes that may indicate a need for PASRR review, complete Part A of the Resident Review Form and document the specific symptoms and behaviors prompting the referral.

• PASRR I score of 2 or 4 for ID/DD/RC: Please email ODHS.

Resident Review

A Resident Review is used for individuals who are already residing in a nursing facility.

If a resident with confirmed SMI experiences significant changes in their cognitive or physical conditions, complete Part A of the Resident Review Form and document the specific symptoms, behaviors, and changes in status prompting the referral.

Requesting a PASRR II Evaluation

If your facility is contracted with Rogue Psych Consultants:

Request that the current RPC provider assigned to your facility complete the PASRR II evaluation. Give the provider the

PASRR I Form or completed Resident Review Form so they can proceed with the evaluation.

If your facility is not contracted with Rogue Psych Consultants:

Please complete our PASRR II Request Form below and fax or securely email the required documentation. We will contact you to schedule an appointment.

3. Patient Records, including:

• Current medication list

• ADLs

• Relevant treatment notes
